Determinants of lung disease progression measured by lung clearance index in children with cystic fibrosis

چکیده
The lung clearance index (LCI) measured by the multiple breath washout (MBW) test is sensitive to early disease in children with cystic fibrosis. While LCI worsens during preschool years fibrosis, there limited evidence clarify whether this continues school age years, and trajectory of progression as modifiable. A cohort (healthy fibrosis) previously studied for 12 months preschoolers were followed (5–10 years). was every 3 a period 24 using Exhalyzer D MBW nitrogen device. Linear mixed effects regression used model changes over time. total 582 measurements 48 healthy subjects 845 64 fibrosis available. majority had elevated at first visits (57.8% (37 out 64)), whereas all but six normal forced expiratory volume 1 s (FEV ) values visit. During course stable (?0.02 units·year ?1 (95% CI ?0.14–0.10). well rate change time period, important determinants FEV 1, age. Preschool major determinant LCI; these findings further support that are critical intervention strategies.

Lung clearance index at 4 years predicts subsequent lung function in children with cystic fibrosis.
RATIONALE The markedly improved life expectancy of children with cystic fibrosis (CF) has created a new challenge, as traditional markers of lung disease are frequently normal in young children. This prevents identification of individuals who may benefit from more aggressive therapy and also obliges large study numbers and prolonged duration for intervention studies. There is an urgent need for...
متن کاملLung clearance index in adults with non-cystic fibrosis bronchiectasis
BACKGROUND Lung clearance index (LCI) is a measure of abnormal ventilation distribution derived from the multiple breath inert gas washout (MBW) technique. We aimed to determine the clinical utility of LCI in non-CF bronchiectasis, and to assess two novel MBW parameters that distinguish between increases in LCI due to specific ventilation inequality (LCIvent) and increased respiratory dead spac...
متن کاملLung Clearance Index and Structural Lung Disease on Computed Tomography in Early Cystic Fibrosis.
RATIONALE The lung clearance index is a measure of ventilation distribution derived from the multiple-breath washout technique. It has been suggested as a surrogate for chest computed tomography to detect structural lung abnormalities in individuals with cystic fibrosis (CF); however, the associations between lung clearance index and early structural lung disease are unclear. OBJECTIVES We as...
متن کاملCystic Lung Disease in a Young man
A 32-year-old man was admitted in the Emergency Department with acute dyspnea secondary to spontaneous pneumothorax. He had history of spontaneous pneumothorax 2 years ago.He was active cigarette smoker (10 pack/year). The family history was unremarkable for lung diseases. Additionally, he had no systemic complaints. The lung HRCT revealed multiple bizarre-shaped cysts distributed in both lungs ...
متن کاملLung clearance index in adult cystic fibrosis patients: the role of convection-dependent lung units.
We investigated the extent of convective ventilation heterogeneity contributing to the observed lung clearance index values in adult cystic fibrosis patients, as the result of two simulated scenarios, either 1) a fixed part of the lungs becoming increasingly less compliant, or 2) an increasingly greater part of the lung being less compliant. In 25 cystic fibrosis patients and 25 matched control...
